Personalized medicine is significantly changing the fundamentals of healthcare. Genetic diagnostic tests are used in various parts of medicine from initial screening to the diagnosis and finallytreatment. Investigations in fields of drug development indicate that oncology may lead other therapeutic areas in the number of targeted therapies on the marketplace as well as in the pipeline, expecting that within a few years, all oncology medications will have a related diagnosis. It has been shown that personal genomics will influence other important therapeutic areas, including the clinical decision making. In conclusion, personalized medicine is just getting started for hematology and respiratory medical therapies, as well as oncology.

Context: This literature review mostly emphasizes on the challenges and barriers for acceptance and implementation of electronic health records (EHRs) in developing countries. Although the willingness of developing countries for acceptance and implementation of EHRs is rising, they will face some challenges and barriers which can slow down their progress due to the multidisciplinary and complexity characteristics of EHRs. Some of these challenges and barriers can be seen in the past and present histories of EHRs acceptance process in developed countries too. Familiarity with these challenges and barriers will help developing countries to have better understanding of these problems towards successful EHRs. The main goal of this survey is to study the challenges and barriers of acceptance and implementation of EHRs in the developing countries and how they can overcome such problems.

Evidence Acquisition: Two databases, Pub-Med and Scopus with Combination of terms such as Electronic Health Records, Developing Countries, Barriers, Challenges, Acceptance, and Adoption were searched. Totally, 514 articles extracted. Some exclusive criteria such as date of publication (year 2000 and above) and having a close relation to the subject of the study (through abstract) were applied. The result was eight articles from Scopus and 12 articles from Pub-Med. Also, 7 articles were extracted by general search in internet. Also, 11 articles with the same search policies were selected from Journal of Health informatics for Developing Countries (JHIDC) with no exclusion criteria.

Results: Developing countries must be fully aware of challenges and barriers in their way towards EHRs. They also need to study the experiences of countries which are successful in acceptance and implementation of EHRs. Developing countries also need to provide infrastructures needed for a successful implementation of EHRs.

Conclusions: Despite the positive effects of EHRs in healthcare services, the acceptance rate of EHRs in developing countries and even developed countries is still low. The developing countries are facing many problems in their way toward implementing successful EHRs. Lack of infrastructures in information and communication technology, cost, and cultural resistance are considered as main barriers for developing countries in their approach to accept and implement EHRs.

Background: PNEA is a neuropsychiatric disorder which has lots of economical and psychological burden not only on patients and their families, but also on society and health systems. Long Term video-EEG Monitoring (LTM) is gold standard way in diagnosis of PNEA.

Objectives: The aim of this study is to investigate occurrence of PNEA in patients admitted to the Razavi epilepsy department and role of LTM in definite diagnosis of PNEA.

Materials and Methods: This study was retrospectively performed and the required information was gathered from database of admitted patients in epilepsy department of Razavi Hospital in 2011. All the patients with final diagnosis of PNEA were included in the study. They were all contacted with telephone thereafter to evaluate their clinical status and compare it with their condition before the clear diagnosis.

Results: The results of this study revealed that, 24-hour LTM associated with noninvasive suggestive provocative techniques in patients with presumptive psychogenic events can lead to definite diagnosis of psychogenic non-epileptic attacks (PNEA).

Conclusions: Definite diagnosis of PNEA not only reduces significantly unnecessary use of AEDs (P = 0.001) and psychotropic drugs, but also increases patients' productivity and emotional wellbeing. In addition, it leads them to accept diagnosis and iollow appropriate treatment.

Background: Breast cancer (BC) is the most common cancer in Iranian women. Studying the mortality statistics is important to monitor the effects of screening programs or the influence of earlier diagnosis on the burden of this chronic disease. Misclassification is still a problem in the Iranian death registry data and about 20% of death statistics are recorded in misclassified categories.

Objectives: The aim of this study is to re-estimate the mortality rate of Breast cancer in the Iran by a Bayesian model in order to exclude the bias due to misclassification.

Materials and Methods: The mortality data were extracted from national death Statistic, which reported or published by the Ministry of Health and Medical Education, from 1995 to 2004. The rate of mortality due to BC [1CD-10; C50] were expressed as the annual rates per/100,000 population in age group (< 15,15 - 49 and > 50 years of age) and also, age standardized rate (ASR) calculated in this study. To correct the misclassification effect, a Bayesian approach was used with Poisson count regression and beta prior.

Results: The results of the Bayesian analysis indicated that there were between 20 to 30 percent misclassified deaths statistic in mortality records which did not reported and re-estimated by this model for BC. Also the rate of BC mortality has increased in recentyears; however it seems that the rate would be leveled off from 2002 to 2004.

Conclusions: The findings revealed a substantial misclassified mortality statistics of BC in the Iranian women. Therefore policy makers in health scope should notice to this unreported data.

Background: Femur fracture is one of the most common fractures in children. Certain parameters such as the patient ’s age, the exact location of fracture, type of fracture, the mechanism of fracture, and the accompanying injuries determine which one of the many treatment techniques be used in each case.
Objectives: We aimed to compare the results of close and open techniques in pediatric femoral shaft fractures fixation (under 9 years old).
Patients and Methods: In this study, the clinical and radiologic results of the femur shaft fracture cases in children below 9 years old who recoursed to Mashhad Imam Reza Hospital. Twenty six cases of femoral shaft fractures in 25 children (14 boys and 11 girls) which could be classified in three different classes; nine upper third (proximal) femoral shaft fractures, 14 middle third (midshaft) femoral fractures, three lower third (distal) femoral shaft fractures. Thirteen fractures (50%) were treated with immediate Spica cast, six fractures (23%) received traction and Spica cast, and surgical fixation with plate was carried out in the remaining seven cases.
Results: After a follow-up interval of at least 5 years, angle deformity was the most common complication observed in 32% of the studied patients. Limb length discrepancy and rotational deformity were observed in 24% and 16% of the cases. Treated with immediate Spica cast proved to have the highest complication prevalence of all (92.2%) and was followed by traction and Spica cast (83.4%) and were observed in patients'' surgical fixation with plate (14.3%).
Conclusion: According to the results, internal fixation with plates can be considered an appropriate technique in cases of pediatric femoral shaft fractures.

Background: Childhood malnutrition still persists in some parts of Iran. Poor breast feeding is proposed as a probable risk factor for malnutrition.
Objectives: In the present study views, experiences and practices of mothers and caregivers about exclusive breast feeding was explored by a qualitative study in Damavand, Iran, a country with a relatively high prevalence of pediatric malnutrition.
Materials and
Methods: Urban and rural mothers who had under-2-year babies attended 11 FGDs. In addition, six FGDs were arranged for caregivers in urban and rural areas. All FGD notes were analyzed and emerging themes were derived and reported.
Results: The findings indicated that although mothers were aware of breastfeeding benefits and regarded it as their responsibility, in few cases, neonates were fed by items other than breast milk. Exclusive breast feeding was not followed precisely. Pregnancy of mothers, lack of knowledge, misinformation and physician instruction were among the reasons why mother may stop breast- feeding. Mothers' and caregivers were aware of the importance of breast feeding in sick children.
Conclusions: Mothers’ knowledge of exclusive breast feeding were satisfactory but their practice was not based on recommendations. As physicians play a role in this matter, the need for specific educational modules for them and other health staff is recommended. Meanwhile, parental education and everyone involved in childcare is a priority so that the problem can be overcome.

Purpose: Razavi hospital plays an important role in the field of medical research and tries to provide a good opportunityfor the researches and pioneers in different medical fields to share their knowledge. Considering the incidence and prevalence of colorectal diseases especially in Iran, these congresses may lead tofind the new solution.

Summary of Presented Findings: The rapid evolution of technology over the last decade has led to the development of new instruments, diag nostic appliances and synthetic or natural materials for tissue repair. The terms "quality of life" and "quality of care", are now familiar in proctology. This has radically changed the rules of this sub-specialty. Now the proctologist must have a familiarity with this technological cornucopia and the use of high-tech materials as well in or der to pursue a correct diagnosis and a proper treatment of the diseasesHence a continuous updating in this field is needed to the state of the art.

Recommendation : Colorectal diseases such as Fistula, Haemorrhoid and Incontinence are common and because of the refractory cases, especially in the case of Fistula, we have to find the efficient treaments. It needs more time and more researches should be done.
